Antibiotic Prophylaxis Consideration in Facial Bone Fracture

Abstract
The routine use of antibiotics in facial bone fracture is widespread despite the lack of scientific evidence for their use. The aim of our study was to investigate the role and effects of antibiotic prophylaxis in facial bone fracture operation. A retrospective study including 69 patients undergoing open reduction was conducted. All the patients had closed fracture with clean wound. We divided the patients into 2 subgroups according to the antibiotics treatment regimen. In group I, 36 patients received a preoperative and postoperative 1st generation cephalosporin IV antibiotics for several days. In group II, 33 patients received preoperative and postoperative 1st generation cephalosporin IV antibiotics only once. All procedures were performed by means of a standard routine approach and fixation methods. The authors compared the incidence of postoperative infections and antibiotic side effects. One patient developed postoperative local wound infections in each group and the difference was not statistically significant. The rate of common antibiotic-induced side effects was significantly higher in the longer treatment regimen group (P=0.01). The results would suggest that a preoperative and postoperative only two times antibiotics treatment regimen is sufficient for prophylaxis of postoperative infections and preferable in the management of their side effects.
